Overview

The Vermont Agency of Human Services is seeking a Reentry Care Coordination Program Manager to lead a statewide Medicaid waiver program supporting sentenced individuals transitioning from incarceration back into the community. This role provides operational leadership to state and regional care coordination teams, strengthens partnerships across corrections, healthcare, housing, and community organizations, and ensures services are trauma-informed, person-centered, and aligned with state and federal priorities. We are looking for a collaborative leader with experience in human services, criminal justice, or healthcare coordination, strong communication and project management skills. Experience with team-based care systems is essential. A master's degree and prior leadership experience are strongly valued. This full-time position is based in Waterbury and requires statewide travel.  Join us in shaping a more supportive and coordinated reentry system for Vermont.

Environmental Factors

Statewide travel required for site visits, meetings, and community engagement.
May include occasional evening or weekend hours to support operations.
Hybrid work available depending on agency policy and operational needs.
Must pass background and security clearance as required by the Department of Corrections or partnering agencies.

Minimum Qualifications

Bachelor’s degree in public administration, Criminal Justice, Social Work, Public Health, or a related field AND three (3) years or more of experience in program coordination, human services, or criminal justice, including supervisory or lead experience.

OR

Five (5) years or more of experience in program coordination, human services, or criminal justice, including supervisory or lead experience.

Preferred Qualifications

Master’s degree in social work, Public Administration, Criminal Justice, or a related field.

Direct experience working with individuals involved in the justice system.

Familiarity with Medicaid and other federal or state-funded human services programs.

Experience in cross-sector collaboration (e.g., justice, housing, behavioral health).

Lived experience with incarceration or reentry is strongly valued.
