Title: DMH Care Manager - Limited Service

Overview
Work as part of the DMH Adult Care Management team with a specific focus around oversight of the mental health Home & Community Based Services (HCBS) statewide Medicaid program. This work will include implementation of conflict free case management with statewide Designated Agencies and HCBS assurances. This Care Manager will review applications for and authorize Medicaid funding of Home & Community Based Services (HCBS) for adults receiving Community Rehabilitation and Treatment (CRT) services through the ten Designated Agencies and Pathways that provide this service statewide. The management of HCBS includes clinical eligibility determination, authorization of Medicaid funding, initial and continued utilization review of service plans, and technical assistance with the DA/SSAs in providing effective community-based programming. The role will also involve collaboration with community providers around complex clinical situations. It may also include developing and providing training for providers on HCBS regulations, clinical eligibility determinations, and measurement and reporting. Familiarity with the mental health system of care specific aspects such as the CRT program, the work of Designated Agencies, HCBS and/or Medicaid funding, intake assessments and eligibility determination, and serious mental illness will all be assets in this position.

Environmental Factors
Duties are performed in a variety of settings, primarily within the Central Office but with need for travel, at times, to other settings such as general hospitals and community mental health centers for which private means of transportation must be available.
Minimum Qualifications
Master's degree in a mental health related field AND four (4) years of clinical experience in counseling, social work, psychology or nursing, including experience with adults with severe emotional disturbance, including two (2) years of experience in a community-based system of care.
Preferred Qualifications
Knowledge of treatment options including inpatient treatment for adults.
Special Requirements
Currently licensed mental health professional such as Licensed Clinical Social Worker, Licensed Psychologist, Licensed Clinical Mental Health Counselor or Licensed Alcohol and Drug Counselor; or possession of Vermont license within six months of hire.
