Overview

The Patient Navigator position supports the You First program, a CDC funded program that provides support to low-income Vermonters for breast and cervical cancer screening and diagnostic services.  This position provides individualized assistance to You First members to help overcome healthcare system barriers and facilitate timely access to quality breast and cervical cancer screening and diagnostics as well as initiation of treatment services when needed.  The patient navigator helps low-income Vermonters access cancer screening through outreach, follow-up, support, education, communication, connection to resources, and medical claims resolution.  Strong written and oral communication skills are a must, along with problem solving, relationship building, empathy, and attention to detail. Health systems and/or clinical social work experience preferred.  This position will work from home until at least March 31, 2021.

AHS BACKGROUND CHECKS:  Candidates must pass any level of background investigation applicable to the position.  In accordance with AHS Policy 4.02, Hiring Standards, Vermont and/or national criminal record checks, as well as DMV and adult and child abuse registry checks, as appropriate to the position under recruitment, will be conducted on candidates, with the exception of those who are current classified state employees seeking transfer, promotion or demotion into an AHS classified position or are persons exercising re-employment (RIF) rights.

Environmental Factors

Duties are performed primarily in a standard office setting. Some travel may be required, for which private means of transportation must be available. Some work outside of normal office hours may be anticipated. Stress may be encountered from deadlines and from operating in a highly publicized and public setting.

Minimum Qualifications

Master's degree in public or business administration or in a human services or health field AND
one (1) year or more of experience at a professional level in health care administration, public or business administration, a Medicaid program or in benefits administration.

 

OR


Bachelor's degree AND three (3) or more years of experience at a professional level in health care administration, public or business administration, a Medicaid program, or in benefits administration.

 

OR

 

Associate's degree AND five years or more of experience at a professional level in health care administration, public or business administration, a Medicaid program or in benefits administration

 

OR

 

High school diploma or equivalent AND seven (7) years or more of experience at a professional level in health care administration, public or business administration, a Medicaid program or in benefits administration.
